Mount with three holes cut

Hello All
Can anyone cut me 2 mounts, each with three holes them?
x1 hole 16mm diameter
x1 hole 43mm diameter
x1 hole 40mm diameter

overall size will be approx 10 inches x 5 inches
layout still to be agreed with my customer.
