cassese underpinner will not pin

Hi, we have a problem with one of our cassese 3099ultra underpinners. When i try to pin a frame the hammer comes up four times in quick succession but fails to add one pin, i don't understand this as its only supposed to add two pins in the first location anyway? any help would be much appreciated.

Two questions

1. is the air pressure up to the recommended pressure?
2. have you been lubricating the o-rings of the foot valve with pneumatic oil? They do dry out.

Hi thank you for your replies, I managed to find the fault yesterday, there is a rectangle thing close to where you adjust the air pressure which has a black box with a wire coming from it screwed to the bottom with a plastic nut thing, the nut had fell off and the black box had slipped off of a copper cylinder. I screwed it back on and it's now working again.
