Hi Kate.
a] Buy a CMC.
b] Get someone with a CMC to cut it for you.
c] Cut the main aperture and title aperture in the top layer and then do it double in the time-honoured stick-the-bottom-layer-to-the-back method. But cut the bottom piece so it doesn't cover the title aperture. Then cut the bottom layer of the title aperture using a separate piece. Try it behind the title aperture in the top layer and see how it fits. If it does fit nicely, fix it in. If not, have another go.
fill in the gaps with scrap bits to make it all level across the back.
Personally, I think it looks cool to have the main window double and the title window errrrr.... undouble.

Re: Double Mounting
I agree, it looks good to have the main window double and the title box single.
Here’s a video showing the method for sticking the two mounts together:
http://www.youtube.com/watch?v=kkR3DvskjBM
Before you stick the inner mount to the outer mount (and after you have cut the title box) you can paste/stick your title in place and then glue/tape the mounts together and cut the inner mount as shown.
It helps when cutting small boxes to make sure you don’t overcut and to actually undercut and finish the corners with a thin razor blade.
Also attached is a useful reminder for cutting multiple windows and the tip about marking each aperture with an X so you don't cut the bevel facing the wrong way.

Re: Double Mounting
Cut your two boards with the bottom one EVER so slightly smaller than the top one (this saves you having to line them up absolutely perfectly) - stick them together very lightly with DS tape, but not so lightly that they may move, just so lightly that they are easy to separate again.
Plot your apertures on the back of the bottom mount and cut them.
Separate the boards - the blade has scored the apertures on the back of the top mount and you've got a template for your top mount.
Using dividers - or a ruler if you don't have any, or even a strip of board the width of the reveal, plot and mark the required distance from those scores - cut.
