Dry mounting consent form

I'm looking at putting together a dry mounting consent form. It needs to be something that customers will sign to say that I have explained all the issues relating to non-reversiblity, the consequences of non-reversibility, together with attendant risks and limits of liability.

I wondered if anyone has already done anything like this and can offer any advice, or has anything they are willing to share.

At the risk of starting another FATG bashing session (I am member BTW) I wonder if the Legal advice offered by the FATG can help with the wording? It seems the sort of thing that they could offer. I'll ask.

We get our customers to sign at the bottom of our order forms to accept our terms and conditions of order and confirm that they authorise us to undertake the order to the framing level discussed and chosen by them and at the price agreed. Anything we are doing to their work is written on the order form, as are any problems with it such as a damaged picture. That way it is all clearly written down and there is no problem with them understanding what we are doing and why.

Works well for us.

Hi Jo,

That would be a nice solution, but my new order form is gonna be cut up after it has been printed, so the customer does not get an A4 size piece of paper, but something smaller.

The form and all the information will be printed out using a laser printer onto some very nice mid grey card. Because the form is created in Excel, I only type each piece of information in once, but that same information gets copied to other boxes on the form as required. After cutting up the card, I will have a card file copy, a working copy, a customer copy and a printed label for the back of the frame. The Excel spreadsheet also has some pricing formulas and pricing options included.

Hence the extra form.
